Notification For Verification Exercise Of Retirees And Deceased Under Defined Benefit Scheme(DBS) For Batch 13

The Kaduna State Pension Bureau is inviting the below retirees and deceased representatives to come along with the following for verification exercise:

NECESSARY REQUIREMENTS FOR PAYMENT VERIFICATION

RETIREMENT BENEFIT

1. One- year statement of account from the retiree’s bank (October 2020 to Date)
2. Printed BVN slip from the bank
3. Original and two photocopies of Computation Sheet.
4. Two Passport Photographs
5. Original and photocopy of Confirmation of Appointment.
6. Original and photocopy of Approval of Retirement

DEATH BENEFIT

1. Original documents of the deceased related to his/her employment in the State or Local Government.
2. Original Account Statement of the deceased from 2 years before to 1 year after his/her death.
3. Original and two copies of Letter of Administration.
4. Original Statement of Estate Account from bank/recent statement of Estate Account
5. Original Computation Sheet of the deceased
6. Two passport photographs of next of kin(s)
7. Original and photocopy of valid means of identification of the next of kin(s) such as National ID Card, Voter’s Card, International Passport or Driving License.
8. Picture of the Deceased
